Administering Medication at School
The medication administration authorization form must be completed fully in order for the school to administer any medication. A new form must be completed at the beginning of every school year, for each medication and each time there is a change in dosage or time of administration of medication. An adult must bring in any form of medication to the school nurse or the school office, along with the form below. All prescription medications must be in the container given to you by the pharmacist or prescriber, and non-prescription medication must be in the original container with the label intact. Parents have you ever wondered if your child has Scoliosis? Well now there is an APP for that. The Schriners Hospital has created an APP so you as a parent can check your child for scoliosis. Just follow the link and it will take you to their story and how to get the APP.

Parents: The Centers for Disease Control (CDC) is warning that this year Influenza season if one of the most deadly for children that they have seen in several years. Please know the symptoms and signs to watch for. I have a link here to take you to the CDC website and it has information on influenza, and when to see your doctor.

Immunization Records
Parents/Guardians: We will once again have access to Wyoming's state immunization data base. If you would like me to be able to look up your child's immunizations on that data base we must first have a signed permission form returned for each of your children. You can find the form at:
